PPO Health Plan
– You pay a portion of the cost for your elected level of coverage
– You can choose your own doctor
– If you use a network provider, there is no deductible
Pharmacy – Specialty Prescriptions
Some self-medications self-administered specialty drugs, such as injectable, infused, oral or inhaled drugs must be purchased from a participating specialty pharmacy. Florida Blue offers you two choices: Accredo Specialty or CVS Caremark.
Pharmacy – Retail & Delivery
Prime Therapeutics offers a network of thousands of participating pharmacies. Using one of these pharmacies and choosing generic medicines when possible will maximize your savings. Out-of-network pharmacy expenses are not covered.
