Four ways you can receive the $200 Flex Dollar COVID-19 Vaccine Incentive!

Currently, all benefits eligible employees receive an annual $250 Flex Dollar credit that is automatically used to reduce your premium costs by $12.50 (20 pay periods) or $10.42 (24 pay periods) each pay period. For Plan Year 2022 only, benefits eligible employees may qualify for an additional  $200 Flex Dollar COVID-19 Vaccine Incentive, increasing your total flex credit to $450, for being fully vaccinate against COVID-19 by October 29, 2021. In order to qualify you must show proof of vaccination during the Open Enrollment period. Fully vaccinated means you have received the second dose of the Moderna or Pfizer vaccine, or the single dose of the Johnson and Johnson. To receive this incentive, you must select one of the enrollment options below. Failure to complete one of the below processes will result in you not receiving the incentive.

Call Center Appointment

You can schedule a call center appointment to attest that you are fully vaccinated, enroll & review your benefits.

Schedule an appointment now and upload COVID-19 Vaccine Card once you receive the confirmation link.

NOTE: Online and telephonic enrollments will remain in pending status until they are verified by a representative. Also, employees meeting with an enrollment counselor will need to complete and sign an affidavit during enrollment session.

Medical or Religious Exemptions for COVID-19 Vaccination Incentive: You may still qualify for the additional Flex Credit without being vaccinated if you receive a medical or religious COVID-19 vaccine exemption. If you are seeking a medical or religious exemption you must complete one of the 4 options above and submit the COVID-19 Vaccination Exemption Request. Please email the District’s Office of Equity & Inclusion/Professional Standards at ADAExemptionRequests@duvalschools.org to request a medical or religious exemption form. The exemption request must be completed in its entirety and e-mailed to ADAExemptionRequests@duvalschools.org by October 29, 2021. If you are unable to e-mail the completed form, you may fax it to 904-390-2468, mail it to 1701 Prudential Drive, Rm 509, Jacksonville, FL 32207, or bring it in person to the Office of Equity & Inclusion/Professional Standards, located at the District’s Administrative Offices building, on any workday between 7:30 a.m. – 4:30 p.m. Failure to complete and submit the request by the deadline will result in denial of the incentive.

You can get vaccinated at one of our school locations!

The District will also be hosting the following community vaccine events:

  1. Saturday, September 18th, 9 a.m. to 12 p.m. at Riverside High School
  2. Saturday, September 25th, 10 a.m. – 1 p.m. at the District Office Parking Lot
  3. Saturday, October 23rd, 10 a.m. – 1 p.m. at the District Office Parking Lot

Please make sure to bring your DCPS ID and insurance card to vaccination events. Take the necessary steps now to take advantage of this incentive!
